PRIVATE PAY & OUT-OF-NETWORK INSURANCE
Being out of network with an insurance company does not mean that the insurance company will not cover some or all of the services rendered. We are able to provide a Superbill for those who have insurance that we are not in-network with.
A superbill contains patient and company information with a list of services provided on the date of treatment. The superbill can be submitted by the patient or guardian to an out-of-network insurance company to seek reimbursement for those services. We encourage all patients or families to contact their insurance provider to learn what out-of-network benefits they have.
Payment for service is due at the time the evaluation or treatment is provided.
Patients, or legal guardians, are responsible for co-pays, co-insurance, and/or deductibles as well as any costs or fees not covered by their insurance.
We accept payment in the forms of cash, credit and personal checks.
